ABSTRACT

Abstract Objective: To assess the knowledge, dental anxiety, and expectations regarding dental services during the COVID-19 pandemic. Material and Methods: The respondents were Indonesian citizens above 18 years old. An online Google survey was administered using a structured questionnaire with a snowball sampling technique. Survey items comprised knowledge related to COVID-19, dental anxiety assessed using the modified DAS (Dental Anxiety Scale) and expectations regarding dental services using four dimensions of dental service quality. All questionnaires were tested for reliability and indicated acceptable and good agreement. The data were analyzed descriptively. Results: A total of 553 responses were analysed. Most respondents were female (72.9%), 76.7% knew of recommendations to postpone dentist visits and 86.8% knew methods of preventing COVID-19 transmission. More than 70% of respondents knew the precaution procedures in the dental office during COVID-19, and only 27.9% had moderate-severe anxiety. Most respondents' expectations regarding dental services during the pandemic era were related to the quality domain of reliability and responsiveness. Conclusion: Respondents knew about COVID-19 transmission and prevention, emergency conditions warranting a visit to the dentist and the procedures used at the dental office. Most respondents stated that they were not anxious about visiting a dentist during the pandemic. The respondents expect the dentist to provide sufficient information to improve oral health and treatment plan.

ABSTRACT Objective: To investigate the association between emotional and behavioural problems and dental fear/anxiety (DFA) in children aged four to 12 years treated at a clinic in southern Brazil. Material and Methods: In this cross-sectional study where mother-child dyads were interviewed, emotional and behavioural problems were investigated using the Strengths and Difficulties Questionnaire (SDQ) (considering five subscales). Children's DFA was evaluated through the Venham Picture Test. For each SDQ subscale, Poisson regression model was explored. Prevalence ratios (PR) were estimated, considering a significant level of p ≤ 0.05. Results: Overall, 128 children participated in this study. Most children were female (54.7%) and aged between 7 and 9 years (39.8%). The prevalence of emotional problems was 47.7% and behavioural problems were 46.1%. The prevalence of DFA was 18.8%. Children with emotional problems had a 2.3 higher prevalence of DFA (95%CI 1.06-5.04). In general, behavioural problems were not associated with DFA (95%CI 0.84-3.34) only when conduct problems were considered (2.20; 95%CI 1.02-4.70). Conclusion: Children aged between 4 and 12 years who present emotional and conduct problems tend to show higher DFA.

ABSTRACT Objective: To evaluate the behaviour and stress of children undergoing restorative treatment with and without sedation. Material and Methods: Participants were 14 healthy children aged between 2.5 and 6 years and with a history of dental behavioural management problems. In the dental treatment visit, the child was treated with non-pharmacological techniques, and in the second, moderate sedation was added. The child received the same procedure performed by a paediatric dentist in both visits: composite resin restoration using local anaesthesia and rubber dam isolation. In both visits, saliva was collected at the children's arrival at the dental clinic, during local anaesthesia and at the end of treatment. The visits were filmed for later analysis of behaviour according to the Ohio State University Behavioural Rating Scale. Results: About 78.5% of children improved their behaviour from the first to the second visit. The salivary cortisol curve of the first visit was maintained in the second visit for 21.4% of children but varied in the remaining participants. Conclusion: Most children presented better behaviour and less stress when sedation was added to non-pharmacological techniques during dental care.

Background: Dental anxiety is a source of problems in the dental treatment that can lead to dental avoidance. Objective: Determine and compare the factors associated with dental anxiety in Peruvian parents. Material and Methods: An observational, descriptive and cross-sectional study was carried out. The sample consisted of children from 3 to 6 years old and their companions, who attended the Instituto Nacional de Salud del Niño in Lima, Peru. The Corah anxiety scale, the dental fear questionnaire and the Frankl scale were used to assess anxiety, parental dental fear, and children's collaboration. The following variables were also evaluated: negative dental experience, treated tooth and treatment of the children. Chi-square statistical tests, Mann-Whitney U and the binary logistic regression model were used. Results: The sample consisted of 325 children and their companions. An association was found between gender (OR = 2.456; 95% CI: 1.037 - 5.818), child collaboration (OR = 0.044; 95% CI: 0.044 - 0.543) and maternal anxiety (p<0.05). Furthermore, there was an association between dental fear (OR = 3.569; 95% CI: 1.136 - 11.218), child collaboration (OR = 0.023; 95% CI: 0.003 - 0.172) and paternal anxiety (p<0.05). Conclusion: The factor associated with maternal dental anxiety was the sex of the child, and on the father's side, it was the dental fear experienced by them. Moreover, the component affiliated with both parents was the child's collaboration.


Introducción: La ansiedad dental es una fuente de problemas en el tratamiento odontológico que puede conducir a la evitación dental. Objetivo: Determinar y comparar los factores asociados a la ansiedad dental en padres peruanos. Material y Métodos: Se realizó un estudio observacional, descriptivo y transversal. La muestra estuvo compuesta por niños de 3 a 6 años de edad y sus acompañantes que asistieron al Instituto Nacional de Salud del Niño en Lima, Perú. Se usó la escala de ansiedad de Corah, el cuestionario de miedos dentales y la escala de Frankl para evaluar ansiedad, miedo dental parental y colaboración de los niños respectivamente. También se evaluaron las siguientes variables: experiencia dental negativa, pieza dental tratada y tratamiento de los niños. Se utilizaron las pruebas estadísticas de Chi-cuadrado, U de Mann-Whitney y el modelo de regresión logística binaria. Resultados: La muestra estuvo conformada por 325 niños y sus acompañantes. Se encontró una asociación entre el sexo (OR =2.456; 95%IC: 1.037 ­ 5.818), colaboración del niño (OR=0.044; 95%IC: 0.044 ­ 0.543) y la ansiedad materna (p<0.05). Se encontró una asociación entre el miedo dental (OR= 3.569; 95% IC: 1.136 ­ 11.218), colaboración del niño (OR= 0.023; 95% IC: 0.003 ­ 0.172) y la ansiedad paterna (p<0.05). Conclusión: El factor asociado a la ansiedad dental materna fue el sexo del niño, y en el padre, fue el miedo dental experimentado por el mismo. El factor asociado a ambos padres fue la colaboración del niño.

Objetivo: O objetivo desse trabalho foi avaliar a associação entre as características das crianças com o estresse apresentado por estudantes de graduação em Odontologia durante o atendimento odontopediátrico. Materiais e métodos: Participaram 48 crianças com idade entre 6 e 10 anos e 37 alunos voluntários do 7º, 8º e 9º semestres de um curso de Odontologia. Foi realizado um questionário sobre aspectos socioeconômicos e também coletados dados sobre comportamento, percepção de dor e ansiedade da criança e do responsável. Ainda, foram coletados dados relacionados aos alunos da graduação: sexo, semestre, nível de estresse, tipo de procedimento realizado. Foram obtidas as frequências absolutas e relativas por meio da análise descritiva dos dados. As comparações nos desfechos de interesse foram feitas utilizando o teste quiquadrado para variáveis dicotômicas e categóricas, teste linear por linear para variável ordinal e teste exato de Fischer para contagens menor do que 5. Foi adotado um nível de significância de 5%. Resultados: Os resultados mostraram que a maioria dos operadores que atenderam crianças não colaboradoras relataram nível de estresse alto (83,33%) (p<0,001). Os operadores que atenderam crianças com nível de ansiedade médio ou alto relataram nível de estresse alto (p= 0,043) e em pacientes com dor, o nível de estresse da maioria dos operadores foi considerado alto (75%) (p=0,001). Conclusão: as características das crianças influenciaram no estresse apresentado pelo aluno operador durante o atendimento odontológico infantil. Crianças ansiosas, não colaboradoras e que estavam sentindo dor durante o atendimento deixaram o operador mais estressado.


Objective: The aim of this study was to evaluate the association between the characteristics of children and the stress presented by undergraduate dentistry students during pediatric dental care. Materials and Methods: Participants were 48 children aged between 6 to 10 years old and 37 volunteer students from the 7th, 8th and 9th semesters of a Dentistry course. A questionnaire on socioeconomic aspects was carried out and data on behavior, perception of pain and anxiety of the child and the guardian were also collected. Also, data related to undergraduate students were collected: sex, semester, stress level, type of procedure performed. Absolute and relative frequencies were obtained through descriptive data analysis. Comparisons in the outcomes of interest were made using the chi-square test for dichotomous and categorical variables, linear by linear test for ordinal variable, and Fisher's exact test for counts less than 5. A significance level of 5% was adopted. Results: The results showed that students who assisted non-cooperative children reported a high level of stress (83.33%) (p<0.001). Students who assisted children with medium or high levels of anxiety reported high levels of stress. (p=0.043) and in patients with pain, the students' stress level was considered high (75%) (p=0.001). Conclusion: The characteristics of the children influenced the stress presented by the student during child dental care. Anxious children, noncooperative and who were in pain during care left the student more stressed.

El tratamiento odontológico sigue siendo percibido en la actualidad como una experiencia atemorizadora para muchas personas. Se han establecido diversos métodos para tratar de evaluar el temor y ansiedad que refieren las personas cuando deben recibir tra-tamientos bucales. Sin embargo, estos no han sido aplicados adecuadamente como parte de la atención clínica cotidiana. Considerando esto, podría suponer-se que el miedo es uno de los factores desencadenan-tes en la deserción de los tratamientos odontológicos, y las personas asistirían solo en el momento de una urgencia. El objetivo del presente estudio fue analizar el afrontamiento de emociones (miedo) en los pacien-tes que concurren a la consulta para la extracción de un tercer molar en la Facultad de Odontología de la Universidad de Buenos Aires (FOUBA). La metodología que se utilizó fue cuantitativa y cualitativa. El diseño fue exploratorio-descriptivo (AU)


Dental treatment is still perceived today as a frightening experience for many people. Various methods have been established to try to assess the fear and anxiety that people report when they must receive oral treatments, however, these have not been adequately applied as part of daily clinical care. Considering this, it could be assumed that fear is one of the triggering factors in desertion of dental treatments and people would attend only at the time of an emergency. The objective of this study was to analyze the coping with emotions (fear) in patients who attend the consultation for the extraction of a third molar at the Faculty of Dentistry of the University of Buenos Aires (FOUBA). The methodology used was quantitative and qualitative. The design was exploratory-descriptive (AU)

Abstract Objective: To evaluate the effect of laughter therapy on reducing anxiety and pain during dental procedures in children 5-7 years of age. Material and Methods: 48 children aged 5-7 years were included in this cross-over double-blinded clinical trial after the parents completed the Screen for Child Anxiety Related Disorder questionnaire (SCARED). After allocation into two groups: laughter intervention (A) and neutral intervention (B), the anxiety as well as pain were determined by Modified Child Dental Anxiety Scale Faces questionnaire (MCDASF) and the Wong-Baker Faces Scale, respectively. Also, the child's behavior during the treatment was recorded using the Sound, Eye, Motor scale (SEM). Data were analyzed by SPSS 21 using Paired t-test, Independent t-test, Chi-square, Mann-Whitney and Wilcoxon's test. Results: The mean score of anxiety in the laughter intervention group (17.42±2.74) was significantly less than (22.06±2.16) in the neutral intervention group (p=0.000) and lower in boys in both groups (p=0.000, p=0.047). The mean pain severity reported by the children in the neutral intervention group (5.33±1.81) was higher than in the laughter intervention group (2.38±1.87; p=0.00) and higher in girls in both groups (p=0.02; p=0.03). Conclusion: The laughter intervention before dental procedures had a significant effect on reducing anxiety and pain during dental treatment (AU).

ABSTRACT Objective: To evaluate the efficacy of pre-cooling and the use of higher gauged needles in reducing pain during local anesthetic infiltration. Material and Methods: We conducted a split-mouth randomized controlled trial among 70 patients who require bilateral maxillary local anesthetic (LA) injections for dental treatment. After applying the topical anesthetic, each participant received four local anesthetic injections, two on buccal and two palatal sides. At each visit, the participants received one buccal and one palatal infiltration based on the randomization. On the buccal aspect, participants received LA with a 26G needle injection on one side (control) and a 31G needle (test) on the contralateral side. On the Palatal aspect, participants either received LA with a 31G needle on one side (control). In contrast, the opposite side was preceded by topical ice application (iced cotton swab) before LA with a 31G needle (test). Both the visits were spaced with a gap of 7-10 days based on the participants' feasibility. Participants were asked to rate the pain on a visual analog scale independently for buccal and palatal LA injections. Results: On the Buccal aspect, the mean pain scores were 2.74 ± 1.26 and 2.11 ± 1.26 for control and test groups, respectively (p=0.002). On the Palatal aspect, the mean pain scores were 4.14 ± 1.49 and 4.3 ± 1.80 for control and test groups, respectively (p=0.295). Conclusion: Significant lower pain scores were reported with higher gauge needles (31G) when compared to traditional (26G) needles on the buccal aspect. No significant difference was seen with pre-cooling the injection site on the palatal aspect when used with higher gauged needles (31G) (AU).

O objetivo deste estudo foi avaliar a validade da Escala de Imagens Faciais (Facial Image Scale; FIS) em pacientes odontopediátricos do Rio de Janeiro, Brasil, por meio da: 1) validade de critério concorrente, medida através do Venham Picture Test ­ versão brasileira; 2) validade de construto, medida através da correlação da FIS com as variáveis idade, sexo, ansiedade percebida pelos pais/cuidadores, classificação socioeconômica (renda), primeira visita ao dentista, tipo de tratamento realizado, uso de anestesia local, experiência prévia e atual de dor de dente, índice de cárie dentária e comportamento durante a consulta odontológica; 3) avaliação qualitativa com 30 crianças de forma a conhecer se o instrumento estava capturando o sentimento da criança em relação à ATO. A forma de aplicação da FIS foi adaptada para o contexto brasileiro por um grupo de quatro odontopediatras brasileiras e uma psicóloga britânica (autora da FIS). O trabalho foi aprovado pelo comitê de ética em pesquisa do Hospital Universitário Pedro Ernesto - HUPE/UERJ. Foram incluídas crianças de três a 12 anos de idade e seus responsáveis, que compareceram ao tratamento odontológico em clínicas de odontopediatria da Faculdade de Odontologia da Universidade do Estado do Rio de Janeiro, do serviço de Odontologia da Policlínica Piquet Carneiro e do Ambulatório Naval da Penha da Marinha do Brasil. Foram excluídas crianças com deficiências e que não quiseram participar do estudo. Foi realizado um estudo piloto com objetivo de avaliar a compreensão da FIS e testar os procedimentos que seriam empregados no estudo principal, em uma amostra não probabilística de 15 crianças/responsáveis. Os resultados sugeriram que a idade poderia influenciar na mensuração da validade da escala e foi decidido realizar um processo de amostragem por cotas, adaptações das instruções de aplicação da FIS e VPT para uma linguagem mais acessível e decisão de realizar o estudo qualitativo. A amostra selecionada por cotas do estudo de validação compreendeu 150 crianças, divididas em grupos de 31 crianças nas faixas etárias 3-4 anos, 5-6 anos, 7-8 anos, 9-10 anos e 26 crianças na faixa etária de 11-12 anos; 65 (43%) eram do sexo feminino; 20 (13,3%) estavam indo ao dentista pela primeira vez; 47 procedimentos invasivos foram realizados no dia da entrevista (tratamento restaurador, procedimentos sob isolamento absoluto, exodontia e/ou tratamento endodôntico); 55 (36,7%) não tiveram experiência de cárie na dentição decídua (ceod=0) e 111 (74%) na dentição permanente (CPOD=0). O coeficiente de correlação de Spearman (rs) com a Venham Picture Test foi moderado (rs= 0,50; p<0,01) e foram encontradas associações estatisticamente significativas (p <0,05) com sexo feminino, ansiedade percebida pelos cuidadores, uso de isolamento absoluto, dor de dente atual e experiência de cárie. O estudo qualitativo encontrou discrepâncias entre a escolha da FIS e o sentimento da criança em relação à ATO em todas as faixas etárias. Os resultados sugerem que a FIS não parece ser capaz de medir de forma válida a ATO em crianças brasileiras


This study aimed to evaluate the validity of the Facial Image Scale (FIS) in pediatric dental patients in Rio de Janeiro, Brazil, based on: 1) the criterion validity, which was measured with the Venham Picture Test - Brazilian version; 2) the construct validity, using the correlation of FIS with the following variables: age, sex, DA perceived by parents/caregivers, socioeconomic classification (income), first visit the dentist, type of treatment performed, use of local anesthesia, previous and current experience of toothache, dental caries index and behavior during the dental appointment; and 3) a qualitative assessment with 30 children to evaluate if the instrument was capturing the child's feeling regarding DA. The form of application of the FIS was adapted to the Brazilian context by a group of four Brazilian pediatric dentists and a British psychologist (author of the FIS). The work was approved by the research ethics committee of the Pedro Ernesto University Hospital ­ HUPE/UERJ. Children aged 3 to 12 years old and their guardians who attended a dental appointment in the pediatric dentistry clinics of the Faculty of Dentistry of the Rio de Janeiro State University, the dental service of the Policlínica Piquet Carneiro Polyclinic and Penha's Naval Ambulatory of the Brazilian Navy were included. Children with special needs and who did not want to participate in the study were excluded. A pilot study was carried out to assess the understanding of FIS and test the procedures that would be used in the main study, in a non-probabilistic sample of 15 children/guardians. The results suggested that age could influence the measurement of FIS validity and it was decided to carry out a sampling process by quotas, adaptations of the FIS and VPT application instructions for a more accessible language and decision to carry out the qualitative study. The sample selected by quotas of the validation study comprised 150 children, divided into groups of 31 children aged 3-4 years, 5-6 years, 7-8 years, 9-10 years, and 26 children aged 11-12 years; 65 (43%) were female; 20 (13.3%) had their first dental appointment; 47 invasive procedures were performed on the day of the interview (restorative treatment, procedures under rubber dam, extraction and/or endodontic treatment); 55 (36.7%) had no experience of caries in the primary dentition (dmft=0) and 111 (74%) in the permanent dentition (DMFT=0). Spearman's correlation coefficient (rs) with the Venham Picture Test was moderate (rs=0.50; p<0.01); statistically significant associations (p<0.05) was found with female gender, anxiety perceived by caregivers, use of rubber dam, current toothache, and caries experience. The qualitative study found discrepancies between the choice of FIS and the child's feelings towards DA in all age groups. The results suggest that FIS does not seem to be able to measure DA in Brazilian children.

Objetivo: Evaluar la eficacia de la musicoterapia para la reducción de la ansiedad dental en niños entre 5 a 12 años con discapacidad durante la atención odontológica. Materiales y métodos: Estudio cuasi experimental. Se evaluaron 40 pacientes, divididos aleatoriamente en 2 grupos: grupo de expuestos y no expuestos a musicoterapia. El nivel de ansiedad fue registrado por la escala de imagen facial antes y después de la atención clínica, el comportamiento durante la atención a través de la escala de Frankl y el nivel de ansiedad de los padres a través de la escala modificada de Corah. Se registraron las funciones vitales de ambos grupos antes y después de dicha atención. El análisis de los datos se realizó análisis descriptivo (media ± desviación estándar) y análisis inferencial: Test de T-Student y Chi cuadrado (p<0,05). Resultados: Se encontró que la presión arterial inicial/ final en el grupo expuesto fue (Sístole: 108.75 ± 8.56/ 95.50 ± 6.86, Diástole: 67.75 ± 7.15 / 57.75 ± 6.38), y en el grupo no expuesto (Sístole: 101.75 ± 8.77/ 110.15 ± 13.31, Diástole: 59.20 ± 6.08) respectivamente, y la frecuencia cardiaca inicial/final en el grupo expuesto (72.30 ± 7.18 / 63.95 ± 6.12); y en el grupo no expuesto (66.50 ± 9.88 / 73.90 ± 11.46). Se encontraron diferencias estadísticamente significativas en los valores de la presión arterial (sístole, p<0,001 y diástole, p=0,007) y frecuencia cardiaca final (p=0,001), así como una reducción de los niveles de ansiedad final en el grupo expuesto a musicoterapia (p=0,001).

Materiais e métodos: Estudo quase experimental. Avaliou-se 40 pacientes divididos aleatoriamente em dois grupos: grupos de expostos e não expostos a musicoterapia. O nível de ansiedade foi registrado pela escala de imagem facial antes e durante o atendimento odontológico, o comportamento durante o atendimento foi através da escala de Frankl e o nível de ansiedade dos padres através da escala modificada de Corah. As funções vitais foram registradas antes e depois do atendimento. Resultados: A pressão arterial inicial/final no grupo exposto foi (Sístole: 108.75 ± 8.56/95.50 ± 6.86, Diástole: 67.75 ± 7.15/ 57.75 ± 6.38), e no grupo não exposto (Sístole: 101.75 ± 8.77/110.15 ± 13.31, Diástole: 59.20 ± 6.08) respetivamente, e a frequência cardíaca inicial/final no grupo exposto (72.30 ± 7.18/ 63.95 ± 6.12); e no grupo não exposto (66.50 ± 9.88 / 73.90 ± 11.46). Se encontraram diferenças estatisticamente significativas nos valores da pressão arterial (sístole, p=0,000 e diástole, p=0,007) e frequência cardíaca final (p= 0,001), assim como na redução dos níveis de ansiedade final no grupo exposto à musicoterapia (p=0,001). Conclusão: O uso da musicoterapia diminuiu significativamente os níveis da ansiedade durante o atendimento odontológico nos pacientes com discapacidades


Objective: To evaluate the effectiveness of music therapy in reducing dental anxiety in children with disabilities between 5 and 12 years old during dental care. Materials and methods: Quasi-experimental study. 40 patients were evaluated, randomly divided into 2 groups: a group exposed to music therapy and a group not exposed to it. The levels of anxiety were registered using the Facial Image Scale before and after the dental care, the behaviour during dental care using the Frank Scale and the level of anxiety of the parents using the Modified Corah Scale. The vital functions of both groups were registered before and after the dental care. The data analysis was performed using descriptive analysis (media ± standard deviation) and inferential analysis: T- Student test and Chi square (p<0,05). Results: It was found that the initial/final blood pressure in the exposed group was (Systole: 108.75 ± 8.56/ 95.50 ± 6.86, Diastole: 67.75 ± 7.15/ 57.75 ± 6.38), and in the unexposed group (Systole: 101.75 ± 8.77/ 110.15 ± 13.31, Diastole: 59.20 ± 6.08), and the initial/final heart rate in the exposed group was (72.30 ± 7.18/ 63.95 ± 6.12); and in the unexposed group (66.50 ± 9.88/ 73.90 ± 11.46). Statistically significant differences were found in the values of blood pressure (systole, p=0,000 y diastole, p=0,007) and heart rate (p= 0,001), as well as a reduction in the levels of anxiety in the exposed group to music therapy (p=0,001). Conclusion: The use of Music therapy significantly reduces levels of anxiety during dental care in patients with disabilities.

Objetivo: efectuar una revisión sistemática sobre la efectividad de las técnicas de manejo de la conducta en niños sistémicamente sanos, en la consulta de Odontopediatría. Métodos: Investigación descriptiva, retrospectiva, de estudios observacionales y ensayos clínicos aleatorios controlados que evaluaron la efectividad de las técnicas en niños y adolescentes de 03 a 18 años de edad. Resultados: criterios de inclusión artículos en inglés y español, publicados desde 2014 al 2019. El análisis de la calidad metodológica de los estudios se realizó mediante la guía Strobe y CASPe. Se identificaron términos de búsqueda en inglés en el tesauro MeSH en la base de datos Pub­M/Medline, Scopus /Elsevier, español, se utilizaron descriptores de las Ciencias de la Salud (DeCS/BIREME) en la base de datos SciELO.org Redalyc.org, Latindex, Dialnet y google académico. Hubo homogeneidad en los estudios respecto a la selección de escalas para medir ansiedad y miedo en los niños al recibir tratamiento preventivo, curativo o restaurador. Los estudios evaluaron mayoritariamente técnicas de distracción aplicadas con recursos audiovisuales para determinar efectividad sobre el comportamiento de los niños en el consultorio odontológico. También reportaron disminución de la ansiedad y miedo, después de la aplicación de técnicas de distracción, e informaron que hubo cambios de conducta de pacientes ansiosos y negativos a pacientes positivos y colaboradores. Conclusiones: estudios mostraron una insuficiente calidad en sus diseños, lo que no permitió emitir juicios respecto a la contundencia y solidez de la evidencia sobre la efectividad de las técnicas aplicadas.

Aim: to conduct a systematic review of the effectiveness of behavior management techniques in systemically healthy children, in the Pediatric Dentistry office. Methods: Descriptive, retrospective, observational studies and randomized controlled clinical trials that evaluated the effectiveness of the techniques in children and teenangers aged 03 to 18 years. Results: The inclusion criteria were articles in English and Spanish, published from 2014 to 2019. The analysis of the methodological quality of the studies was carried out using the Strobe and Consort guide. English search terms were identified in the MeSH thesaurus in the Pub¬M / Medline, Scopus / Elsevier database, and in Spanish, descriptors of Health Sciences (DeCS / BIREME) were used in the SciELO.org database, Redalyc.org, Latindex, Dialnet and Google Scholar. There was homogeneity in the studies regarding the selection of scales to measure anxiety and fear in children when receiving preventive, curative or restorative treatment. The studies mostly evaluated distraction techniques applied with audiovisual resources to determine their effectiveness on the behavior of children in the dentistry office. They also reported decreased anxiety and fear, after the application of distraction techniques, and reported that there were behavioral changes from anxious and negative patients to positive and collaborative patients. Conclusions: The studies showed insufficient quality in their designs, which did not allow judgments regarding the strength and strength of the evidence on the effectiveness of the techniques applied.

ABSTRACT Objective: To analyze children's and parents' oral health-related quality of life (OHRQoL) and children's dental fears before and after the dental rehabilitation under general anesthesia (DRGA). Material and Methods: 104 parents and their 3 to 13-year-old children (5.90 ± 2.42) who received DRGA were surveyed before and after DRGA. The children were divided into two groups: Group 1 - healthy children (n=43) and Group 2 - children with medical problems (n=61). After recording their socio-demographic information, parents completed a self-administered questionnaire named Early Childhood Oral Health Impact Scale (ECOHIS), which includes two main parts - Child Impact Section (CIS) and Family Impact Section (FIS). On the other hand, the children received a dentist-administered questionnaire named Children's Fear Survey Schedule-Dental Subscale (CFSS-DS) and Frankle Behavior Scale (FBS). For statistical analyses, Wilcoxon Signed-Rank, Mann Whitney-U, Kruskal-Wallis, and Spearman's Correlation tests were used. Results: A statistically significant decrease in all CIS, FIS, ECOHIS and CFSS-DS scores was observed after DRGA (p<0.01). This decline was greater in healthy children than in children with systemic problems (p<0.01). Conclusion: Children's and parents' OHRQoL showed better results after DRGA. The decreases in dental anxiety in children were observed after DRGA.

ABSTRACT Objective: To evaluate the efficacy of pre-cooling and the use of higher gauged needles in reducing pain during local anesthetic infiltration. Material and Methods: We conducted a split-mouth randomized controlled trial among 70 patients who require bilateral maxillary local anesthetic (LA) injections for dental treatment. After applying the topical anesthetic, each participant received four local anesthetic injections, two on buccal and two palatal sides. At each visit, the participants received one buccal and one palatal infiltration based on the randomization. On the buccal aspect, participants received LA with a 26G needle injection on one side (control) and a 31G needle (test) on the contralateral side. On the Palatal aspect, participants either received LA with a 31G needle on one side (control). In contrast, the opposite side was preceded by topical ice application (iced cotton swab) before LA with a 31G needle (test). Both the visits were spaced with a gap of 7-10 days based on the participants' feasibility. Participants were asked to rate the pain on a visual analog scale independently for buccal and palatal LA injections. Results: On the Buccal aspect, the mean pain scores were 2.74 ± 1.26 and 2.11 ± 1.26 for control and test groups, respectively (p=0.002). On the Palatal aspect, the mean pain scores were 4.14 ± 1.49 and 4.3 ± 1.80 for control and test groups, respectively (p=0.295). Conclusion: Significant lower pain scores were reported with higher gauge needles (31G) when compared to traditional (26G) needles on the buccal aspect. No significant difference was seen with pre-cooling the injection site on the palatal aspect when used with higher gauged needles (31G).

ABSTRACT

Dental fear and anxiety arise from several factors, among them, exposure to previous traumatic experiences with needles, injections and instruments that can cause discomfort. Anxious patients tend to reschedule appointments, avoid treatments and are only available for visits at the dentist when painful symptoms appear, so delayed treatment tends to be more invasive and cause even more emotional damage to the patient, consequently aggravate this feeling and, in turn, create a vicious cycle in which avoiding treatment only makes the oral health condition even worse. It is up to professionals of Dentistry to identify anxiety in patients and make use of resources to manage this anxiety in order to enable an effective dental treatment with less emotional damage to the patient. The aim of this study is to evaluate, by means of a integrative literature review, the importance of the study of Psychology by the dental professional and to expose the techniques for coping with dental anxiety, based on scientific evidences.


O medo e a ansiedade odontológica surgem por diversos fatores, entre eles, a exposição à experiências traumáticas anteriores com envolvimento de agulhas, injeções e instrumentos que possam causar incômodo. Os pacientes ansiosos tendem a adiar consultas, evitam os tratamentos odontológicos e só se dispõe às visitas ao dentista quando aparecem sintomas dolorosos, logo o tratamento tardio tende a ser mais invasivo causando ainda mais prejuízo emocional ao paciente, consequente agravo a tal sentimento e, por sua vez, origina um ciclo vicioso no qual evitar o tratamento agrava ainda mais a condição de saúde bucal. Cabe aos profissionais da Odontologia identificar a ansiedade nos pacientes e fazer uso de recursos de manejo dessa ansiedade de modo a possibilitar um tratamento odontológico eficaz e com menor prejuízo emocional ao paciente. Com este estudo pretende-se avaliar por meio de uma revisão integrativa de literatura, a importância do estudo da Psicologia pelo profissional da Odontologia e expor algumas técnicas de enfrentamento da ansiedade odontológica, com base em evidências científicas.

ABSTRACT

En los últimos meses la población del mundo ha estado sometida por la pandemia de COVID-19 a un reto emocional causado por el riesgo de pérdida de la salud (incluida la muerte) y del riesgo real o potencial de la alteración de la estabilidad socioeconómica personal y global. Por otro lado, los individuos reciben diariamente información de diversa valía en relación a veracidad y oportunismo, lo que los expone a incertidumbre respecto a la toma de decisiones relacionadas con el bienestar o asociadas con aspectos laborales y productivos. Debe observarse que esto está sucediendo con: pacientes, empleados, proveedores y profesionistas en el modelo odontológico. El miedo es la respuesta prevalente que induce este conjunto de estímulos amenazantes, en el que de manera secundaria se manifiestan otros estados emocionales, sentimentales y de conducta. Contrarrestar el miedo en todos los actores del concepto práctica dental debería ser la estrategia en la recuperación de la actividad odontológica en esta nueva realidad, siendo la palabra clave, seguridad; misma que no debe estar representada exclusivamente por la implantación de medidas sanitarias, también de manera muy principal por la comunicación y la estimulación positiva de emociones y sentimientos que alcancen la mente afectiva no sólo de los pacientes, sino también de los miembros del grupo de trabajo ­mente inconsciente­(AU)


In recent months the world's population has been exposed by the pandemic advent of COVID-19 to an emotional challenge caused by the risk of health disturbance, death encompassed, and the real or latent risk of disruption of personal and global socioeconomic steadiness. Individuals on the other hand, receive day-to-day information of varying connotation in relation to truthfulness and opportunism, exposing them to decision-making uncertainty regarding welfare, as well employment and productive subjects. At the dental scheme, this is happening with: patients, employees, providers and professionals themselves. The widespread reaction that brings this myriad of threatening stimuli is fear, where other emotional, sentimental and behavioral expressions, secondarily occur. In this new reality, disparate the fear in all and any participant of the dental practice concept should be the strategy applied for dental activity recovering; being the key word, safety. Concept that should not be represented exclusively by the implementation of sanitary actions and warnings, but in a conspicuous manner by communication of this safety concept and the induction of positive stimuli by means of emotions and feelings that could reach the affective mind of patients, and the working group ­unconscious mind­ (AU)

INTRODUCCIÓN: La ansiedad dental impacta profundamente en la salud oral de un paciente, determinando el pronóstico y adherencia a tratamiento, sin embargo, existe poca información de instrumentos validados que evalúen este constructo. El OBJETIVO de este estudio es evaluar las propiedades psicométricas del Dental Anxiety Scale versión en español en adultos en servicios de atención primaria de la Región Metropolitana, Chile. MATERIAL Y MÉTODO: Estudio transversal descriptivo aplicado a 169 adultos acompañantes de un servicio de atención primaria. La validez de contenido se determinó con metodología Delphi, la validez de constructo a través del análisis factorial confirmatorio y la confiabilidad a través de análisis de consistencia interna mediante Alfa de Cronbach. RESULTADOS: El juicio de expertos no modificó la versión en español. El análisis factorial confirmó la presencia de un solo factor, el cual explicó el 65,44% de la varianza, mientras que la confiabilidad arrojó un alfa de Cronbach de 0,822, considerado bueno. DISCUSIÓN: El Dental Anxiety Scale presentó una adecuada validez de contenido, validez de constructo y buena confiabilidad. Se recomienda su uso para la medición de ansiedad dental en adultos que acuden a servicios de atención primaria en la Región Metropolitana.


INTRODUCTION: Dental anxiety can strongly affect oral health, modifying the prognosis and treatment's adherence. The level of dental anxiety has to be considered when proposing a treatment plan. The AIM of this study is to measure the psychometric properties of the Dental Anxiety Scale in the Spanish version, applied to adults in primary health institutions of the Metropolitan Region, Chile METHODS: 169 adults were recruited. To determine content validity, the Delphi method was used; the confirmatory factor analysis was used to demonstrate the construct validity. Reliability was measured in terms of internal consistency with the Cronbach's alpha coefficient. RESULTS: The experts did not modify the Spanish version of DAS. The factorial analysis confirmed that there was a single factor that accounted for 65,44% of variance. A 0,882 Cronbach's alfa is considered as good reliability. CONCLUSION: Dental Anxiety Scale presents an appropriate construct and content validity, and a good reliability. It is a proper instrument to be used in adults in primary health care centers in the Metropolitan Region.

Abstract Objective: To verify whether recreational interaction with pediatric patient before dental care influences cortisol levels and anxiety. Material and Methods: Twelve children were selected and divided into two groups: 6 children who had not previously received recreational intervention (Group I) and 6 children who received recreational intervention before dental care (Group II). Saliva was collected with Salivettes™ kits before and soon after dental care for the measurement of cortisol. Wilcoxon test was applied to verify the difference in salivary cortisol levels before and after the dental visit in the whole sample, and the Mann-Whitney U test was used to analyze the difference between groups I and II. A significance level of 0.05 was considered. Results: Among patients from Group I, 3 had their cortisol levels increased after dental care, 2 had their cortisol levels decreased, and the other 1 patient remained constant. All participants from Group II had no significant difference in cortisol levels pre- and post-procedure. Mean salivary cortisol level was higher in Group I, pre- and post-procedure, compared to Group II. Conclusion: Pediatric patients who participated in an interactive activity prior to dental care have a lower level of anxiety according to the salivary cortisol levels obtained, compared to patients who did not participate in recreation. The techniques for the dentist to approach the child control the anxiety of the patient and make the procedure calmer and less stressful for both of them.

Abstract Objective The aim of this study was to compare the effects of music at 432 Hz, 440 Hz, and no music on the clinical perception of anxiety and salivary cortisol levels in patients undergoing tooth extraction. Methodology A parallel-group randomized clinical trial was conducted. Forty-two patients (average age: 23.8±7.8 years, 27 women) with a moderate level of anxiety were distributed in three groups: use of music for 15 minutes at a frequency of 432 Hz (n=15), at 440 Hz (n=15) and a control group without music (n=12). The CORAH Dental Anxiety Scale and salivary cortisol levels, estimated by the solid phase enzyme-linked immunosorbent assay (ELISA), were measured and compared before and after the music intervention between groups (two-way ANOVA-Tukey p<0.05, RStudio). Results Significantly lower anxiety level values were observed at 432 Hz (8.7±2.67) and 440 Hz (8.4±2.84) compared to the control group (17.2±4.60; p<0.05). The salivary cortisol level at 432 Hz (0.49±0.37 μg/dL) was significantly lower than 440 Hz (1.35±0.69 μg/dL) and the control group (1.59±0.7 μg/dL; p<0.05). Conclusion The use of music significantly decreased clinical anxiety levels, and the frequency of 432 Hz was effective in decreasing salivary cortisol levels before tooth extraction.

ABSTRACT

La atención odontológica despierta en numerosos pacientes, especialmente en los niños, miedo y ansiedad que pueden originar conductas de rechazo a la misma o dificultar el trabajo del profesional. Para registrar la prevalencia e identificar los miedos más frecuentes en relación a la atención odontológica, se realizó un estudio observacional, descriptivo y longitudinal en un trabajo interdisciplinario con Musicoterapia. Se utilizó la Escala del Miedo (CFSS-DS), Dental Subscale of the Childrens' Fear Survey. Los resultados en un promedio de edades en años 11,41; desvío = 2,80, registraron el 53% en la puntuación 38:39. Corresponde 54.34% al sexo femenino y 45.66% al sexo masculino. El mayor porcentaje de miedo se registró a los 10 años de edad 15.24% y en el sexo femenino 54.34%. En conclusión, registrar y reconocer los miedos dentales ofreció un espacio de reflexión y elaboración de los mismos, con docentes y alumnos de grado, para desarrollar, en un tratamiento interdisciplinario con musicoterapia, estrategias que favorezcan una mejor adherencia de los pacientes al tratamiento odontológico y habilidades inherentes y habilidades sociales inherentes a la relación odontólogo-paciente. El miedo manifiesto a la atención odontológica sin ser resuelto satisfactoriamente puede entorpecer las mejores intenciones del profesional en el desarrollo de su tarea (AU)

ABSTRACT Understanding the possible effects that a patient's psycho-neurobiological processes (such as the nocebo effect) may have on the every-day dental treatments, could assist the clinicians in preventing the onset of adverse events not related directly with the clinical procedure. At the same time, employing pathways to trigger plausible placebo effects could aid the clinician to enhance the outcome of ordinary clinical procedures and the patient's perspective. Identified factors, which could lead both to positive or negative effects, may be present in different ways. Prior personal experiences, second-hand information, alternative medicine, catastrophizing or patient motivation; all could have an indirect effect in the treatment outcome. A well-informed clinician should use such factors to individualize each patient treatment.


RESUMEN Comprender los posibles efectos que los procesos psico-neurobiológicos de un paciente (como el efecto nocebo) pueden tener en los tratamientos dentales diarios, podría ayudar a los clínicos a prevenir la aparición de eventos adversos que no estén relacionados directamente con el procedimiento clínico. Un médico bien informado debe usar dichos factores para individualizar el tratamiento de cada paciente.
